Appropriateness of proposed service establishment and implementation plan for supported transition of long-stay consumers to the facility.

Compliance with the Statement of Requirements including:

i. Proposed arrangements for delivery of the model of care.

ii. Ability to meet and commitment to meeting key performance indicators and regular reporting requirements, including capacity of current systems to support the reporting requirements.

iii. Detailed mechanisms for clinical governance within the service and organisation.

Experience in developing and maintaining partnerships with Local Health District health services (either mental health or general health services).

Experience in good practice with facility design and an appropriate overview of the proposed facility design, either purpose built or fit-for purpose, which is consistent with the Quality of Care Principles 2014, and aligned with the Guiding Principles for designing environments for people with dementia and/or psychiatric illness

Experience, capacity and commitment of provider to deliver the services as outlined in the service models and RFT specifications, including:

i. Proposed staffing structure, qualifications and experience of staff, staff education / training support, and service leadership.

ii. Experience in providing care to people with behavioural and psychological symptoms related to mental illness and / or dementia, and / or organisational commitment to providing care for this population.

Financial capacity and stability of the organisation, with evidence to support this.

Price, including assumptions regarding funding from various sources, and any added value/ in-kind contributions brought to the service by the proponent organisation.
