NSW Health / Mental Health - Enhanced Adult Community Living Supports - HAC 15/121

 

The subject of this tender is the enhancement of Community Living Supports to strengthen community based service responses to adults with severe mental illness.  The targeted enhancement will:

 

§   increase the level of community living supports to help a further 500 adults across NSW recover and transition to a quality life in the community;

 

§   deliver a service model that is individualised and flexible;

 

§   be based on the HASI in the Home model in that it will target individuals who have existing stable housing rather than impose pressure on the constrained availability of social housing; and

enable the provision of supports to commence when individuals are still in inpatient care, to better support a successful transition to the community.
