HealthShare NSW, on behalf of Central Coast Local Health District, invites tenders for their Care Awaiting Pathway Program (CAPP).

As part of its Winter Strategy the CCLHD wishes to partner with Residential Aged Care Facilities (RACF) to fund access to beds to accommodate patients who are suitable for discharge and awaiting placement in a RACF.

The CCLHD would like to identify a competent, qualified and experienced RACF providing similar services in a professional and ethical manner willing to establish a strong working partnership to assist with providing care and treatment to the CCLHD discharged patients and invites established RACF in this market to submit detailed proposals for the Services we require, which you feel delivers the best Clinical Care Services, Support Services and Consumables for both parties.
