The Transitional Aged Care Program (TACP) in NSW jointly funded by the Australian Government Department of Health (DOH) and NSW Ministry of Health (MOH). Transition Care Program provides flexible, short-term support and active management for older people at the interface of the acute/sub-acute and residential care sectors. The program is goal oriented, time-limited and targets older people at the conclusion of a hospital episode who require more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity, and finalise their longer term care arrangements. Transition Care can be provided in either a community or a residential setting.
The Objective of this Request for Tender (RFT) is to contract one or more Approved Aged Care provider(s) to operate community transition care, named TRANSPAC CC, for ninety one (91) flexible care places (with the ability to surge to ninety six (96) flexible care places) for eligible older people.
The proposed contract period will be from 1 November 2019 to 31 October 2022, with an option of two (2) twelve (12) month extensions.
