Scheme Details
The NSW Environment Protection Authority (EPA) is the state’s primary environmental regulator. The EPA works with emergency services and partner agencies in responding to emergencies in order to reduce pollution, waste and adverse impacts on the environment.
The EPA is seeking to pre-qualify service providers with the ability to deliver the following services;

Collection and transport of liquid and hazardous waste;

Treatment and Disposal of liquid and hazardous waste;
-
Supporting and ancillary works and/ or services.
Service providers who qualify and are successful with their application will be placed on a non-ranked prequalification scheme which will be used by the EPA, emergency services and partner agencies, within NSW, in the event of an emergency which involves liquid and/or hazardous waste.
The use of the prequalification scheme will be dictated by the requirements of each specific emergency such as urgency, location and supplier availability at that time.

Vendor Instruction
Vendors are required to answer all the questions in this online application and provide the information requested.

Applications must be supported with relevant and completed responses across the following key areas:

relevant information on experience, capacity and capability;

schedule of fees;

insurances;
-
legislation and standards;
-
financial stability and current legal proceedings and conflicts of interest declarations;

agreement to Scheme Rules, which includes the requirement that all customer contracts will comply with the Contract terms and conditions and confirmation that the Applicant is not subject to insolvency, an ICAC inquiry or legal proceedings.

Insurances
Please upload the latest certificate of currency for Professional Indemnity Insurance and provide the insurance expiry date. Note that a minimum coverage of $1 million is required in respect of each claim at time of engagement with the Customer.
Please upload the latest certificate of currency for Workers Compensation as required by all relevant laws of Australia relating to workers' compensation, and provide the insurance expiry date.
Please upload the latest certificate of currency for Public Liability Insurance and provide the insurance expiry date. Note that a minimum coverage of $10 million is required in respect of each claim at time of engagement with the Customer.

- Dangerous Goods Licence/s.
- Environment Protection Licence/s.

If you currently have any legal proceedings pending, please complete and upload the attached template in 'Word' or 'PDF' format.
If appointed, does your organisation have the capacity subject to any insolvency proceedings and has the capacity to pay debts upon the date when they become due in the ordinary course of business.
In support of this disclosure please provide a letter of solvency completed by a nominated company representative that manages the company finances and is CPA/ACA qualified to support your application.
